- Street and No.: Enter the exact address of the church/memorial hall.
- City: Indicate the city in which the church/memorial hall is located.
- Phone: Enter the contact person's phone number or yours.
- Text on card: Write the text you wish to place on the ribbon at this location. Keep in mind that shorter text is more appropriate for the ribbon.
- If you have longer text, we will be happy to transcribe it into a condolence message.
- Instructions for us: In this section, please include the exact start time of the funeral ceremony and possibly other requirements.
